You have occasional chest discomfort, breathlessness or a racing heartbeat. You get an electrocardiogram (ECG), and the report comes back normal. Naturally, you feel relieved. A normal ECG is reassuring, but it is not a complete assessment of cardiovascular health. It captures the heart’s electrical activity at the time of recording and does not assess the heart’s structure, valves, pumping function or coronary arteries. Intermittent symptoms such as palpitations may also not occur during the test itself. The ECG should therefore be interpreted alongside the patient’s symptoms, clinical examination and risk factors, with additional tests recommended when clinically indicated.
An ECG Is an Important Starting Point
An electrocardiogram records the electrical signals that control the heartbeat. It shows the heart rate and rhythm and can help doctors identify certain cardiac abnormalities. When a rhythm disturbance, a conduction block, evidence of a prior heart attack or an inherited rhythm disorder such as long QT syndrome is present at the time of recording, an ECG can detect it reliably. This makes it an important first-line test when someone experiences symptoms such as chest discomfort, palpitations, breathlessness or dizziness.
However, a standard resting ECG is essentially a snapshot. It records the heart’s electrical activity during the brief period when the test is performed. If an abnormal rhythm occurs intermittently, it may not be present during that recording. A resting ECG can also appear normal in a patient with significant coronary artery disease, and even in the early stages of a heart attack, before characteristic changes have had time to develop. This is why active chest pain is evaluated alongside blood tests such as cardiac troponin rather than the ECG alone. An ECG also does not directly visualize the coronary arteries or assess factors such as blood pressure, cholesterol or blood sugar.

Different Tests Answer Different Questions
This is why cardiac evaluation is not about choosing one test over another, but using the right investigation for the clinical question. An echocardiogram assesses the heart’s structure, valves and pumping function, while a Holter monitor can detect intermittent rhythm abnormalities that a routine ECG may miss. A stress test evaluates how the heart responds to exertion, while CT coronary angiography, when indicated, provides detailed images of the coronary arteries. The choice of investigation depends on the patient’s symptoms, risk factors and overall clinical assessment.
Your ECG Is Normal. What About Your Risk Factors?
Heart health extends beyond an ECG. Blood pressure, LDL cholesterol and blood sugar are key indicators of cardiovascular risk but cannot be assessed through a routine ECG. High blood pressure can strain the heart, while high LDL and diabetes can contribute to plaque buildup in the arteries, often without causing ECG changes. Smoking, inactivity, excess weight, diet and family history also influence long-term risk. A family history of sudden cardiac death at a young age is particularly significant, as it can point to inherited rhythm disorders that warrant dedicated cardiac evaluation. Cardiovascular disease remains the world’s leading cause of death, claiming around 20.5 million lives globally each year, according to the World Heart Federation.
Symptoms Still Matter
A normal ECG is reassuring, but persistent or recurrent symptoms should not be ignored. Chest discomfort with exertion, unexplained breathlessness, unusual fatigue, fainting or recurrent palpitations warrant medical evaluation. Chest pain at rest, breathlessness with chest pain, or fainting should prompt immediate medical attention rather than a routine follow-up visit. The timing, triggers and duration of symptoms can help guide further assessment. Importantly, a normal ECG does not mean everyone needs additional tests. The goal is to choose the right test for the right clinical question.

Always Look at the Whole Picture
A normal ECG is good news, but it is only one piece of the heart-health puzzle. A complete assessment may also consider symptoms, clinical examination, blood pressure, cholesterol, blood sugar, lifestyle and family history, with additional investigations when clinically indicated.
The goal is not more tests, but the right assessment based on the complete clinical picture.
